1. Your professional background and experience.

2. Why do you wish to serve on the Board of Directors?

3. What specific skills (e.g., human resources, insurance and risk management, commercial real estate, education) would you bring to the Board?

IV. Conflict of Interest Disclosure

Purpose:

This disclosure form is intended to identify actual, potential, or perceived conflicts of interest in accordance with applicable South Carolina law and board policy. Board members must disclose any interests that could improperly influence—or appear to influence—their decision-making.

Definitions: 

Family Member: Any board member’s, officer’s, employee’s spouse, parent, sibling, child, mother-in law, father in-law, son-in-law, daughter-in-law, grandparent, or grandchild. The board further includes in the definition of "family member" an individual claimed by a board member or the board member's spouse as a dependent for income tax purposes.

Do you or any immediate family member have any direct or indirect financial interest, ownership interest, employment relationship, compensation arrangement, or other business relationship with any entity that:

• Does business with the school

• Seeks to do business with the school

• Is otherwise affected by board decisions

2. Gifts, Compensation, or Benefits

Have you or a family member received (or expect to receive) anything of value (gifts, favors, discounts, payments, or opportunities) from any individual or entity that does or seeks to do business with the school?

3. Personal or Professional Relationships

Do you or a family member have a personal, professional, or organizational relationship with any of the following that could influence your role?

• School employee

• Contractor/vendor

• Applicant or partner organization

4. Other Potential Conflicts

Are you aware of any other circumstance that could:

• Create an actual conflict of interest, OR

• Create the appearance of a conflict

5. Duty to Disclose and Recuse

   I understand that:

   • I must disclose conflicts as soon as they arise

   • I must recuse myself from discussions and votes where a conflict exists

   • My disclosure may be recorded in official board mintes
